- You may also request the records directly from your previous practice if you wish to do so.

RVA Pediatrics wishes you the best and wants to make this process as seamless as possible.
- When you have chosen a new provider, download, complete, and provide us with the Medical Records Release Form. This form can be found here.
- You may drop off the medical records release form at any of our offices or you may fax the form to: 804-754-0880.
- We kindly ask for 7-10 business days to process requests. Our medical records are sent to the recipient on a CD disc. Beginning on May 1, 2023, there will be a fee of $30 for all medical records transfers.
